HOW TO ORDER AIRWAY MANAGEMENT PRODUCTS

To order airway management products from Bryan Medical, download or fill in the product order form here for the purchase of all tracheostomy tubes, related protection products, cleaning supplies, medical office examination chairs, chair accessories, and treatment cabinets. Submit the completed form to us via email or fax.
For a custom tracheostomy tube, download the manufacturer’s custom template form, complete the form and submit it with the general order form. Note the information required for standard or customary tubes below.

All Tracheostomy Tubes:

Determine the correct Tracheostomy tube for the patient, including:
  1. Type of Tube
  2. Reference Number
  3. Cuffed or Uncuffed

STANDARD Tracheostomy Tube:

  1. Provide a reference number and description for each tube needed to the Purchasing Department
  2. The Purchasing Department should submit a purchase order directly to Bryan Medical by email (sales@bryanmedical.net) or by fax (513-272-1610). Required information on all purchase orders:
    1. PO number
    2. Product reference number
    3. Quantity
    4. “Bill to” address
    5. “Ship to” address
    6. Contact name/telephone number/email address (for questions).

CUSTOM Tracheostomy Tube

  1. Fill out the appropriate custom template.
  2. Send the custom template to the Purchasing Department and request an order be placed. Time frame is important to state.
  3. Provide the Purchasing Department with a reference number and description for each tube needed.
  4. The Purchasing Department should submit a purchase order WITH TEMPLATE directly to Bryan Medical by email (sales@bryanmedical.net) or by fax (513-272-1610). Required information on all purchase orders:
    1. PO number
    2. Product reference number
    3. Quantity
    4. “Bill to” address
    5. “Ship to” address
    6. Contact name/telephone number/email address (for questions).
